What is Mission 615? It’s a local mission experience that allows our middle school students to see the needs in our own area code and respond in Christ-like love. Space is limited.

COST & REGISTRATION

Student Cost: $130 | $25 Deposit due with Registration

Adult Cost: $40

Cost includes transportation and lunch for your student.

Registration for Mission615 is done through Brentwood’s Focus Mission program.

ADULTS 

Are you interested in leading students at Mission 615? We would love to have you join us! We need leaders willing to help lead students in serving and in a small group setting, as well as adults comfortable driving a 12-passenger van.

PARENT MEETING

Tuesday, May 30 | 5:30 -6:30 PM | Wilson Hall D+E | Brentwood Campus

• During this meeting, we’ll share details about the journey and what your student can expect. This will also be an opportunity for you to make final payments and turn in any required documents ahead of check-in!

LEADER MEETING

Tuesday, May 30 | 6:30-7:30PM | Wilson Hall D+E| Brentwood Campus

REQUIRED FORMS

Please complete and turn in the following forms to the Student Ministry office no later than the parent meeting on Tuesday, May 30th.

REQUIRED FOR EACH STUDENT or LEADER:

• 2023 DIGITAL Student Release Form (*if you have not been on a student ministry trip in 2023, you must submit a new release form for the calendar year)
  –or–
• 2023 Adult Release Form 

• • • *if you have not been on a student ministry trip in 2023, you must submit a new release form for the calendar year)

-AND-

• A copy of your insurance card (this can be emailed to Anna Brackins-Gilmore at [email protected]).

OTHER FORMS:

• 2023 Medication Form – if your student has medication. If needed, please print and turn in at check-in on June 5th.
• 2023 EPIPEN Form – if your student has an EPIPEN is this form required. If needed, please print and turn in  at check-in on June 5.
• 2023  DIGITAL Time Away – if your student will not be taking church provided transportation, and/or will be arriving late or leaving early from Mission615. *This form is due to student ministry office no later than Tuesday, May 30th*

We will be collecting paperwork on the following dates at the Brentwood Campus:

• Tuesday, May 30 | 5:30 -6:30 PM | Wilson Hall D+E | Brentwood Campus

**Digital paperwork + payments can be submitted at any time to the Student Ministry office before May 30**

CANCELLATION POLICY

If you need to cancel, please notify the student ministry office as soon as possible at 615-324-6126 or [email protected]. Due to trip cost already incurred, there will be no refunds for cancellations* after registration closes on May 31, except under special circumstances as determined by the ministers.

*Does not apply to extenuating circumstances, such as death of a family member, COVID required quarantine/sickness or other physical illness.
